Answer# 5   I have issues with billing hyperbaric treatments and wondered if you could help me. I am using 99183 for physician billing and he wants the quantity of 4 billed with it. We are getting reimbursement but only by medicare others such as medicaid are rejecting stating it should be billed as one unit. Please shed some light on this subject for me. Thank you for your help in this matter.  
Answer# 34   Concurrent billing with Evaluation and Management and 99183. Is there a problem doing this? Our Medicare codes carrier demands a re- evaluation every seven-10 days, etc. Can we bill for this and also bill for the 99183? We have heard that this is not legal. For this (E/M) and also the treatment (99183).
 
Answer# 48   What are your protocols for cleaning a monoplace chamber following a treatment of a "clean" patient ie. ORN, CO, DCS and someone with MRSA, open draining wounds, or necrotizing fasciitis, etc.  
Answer# 67   What is the level of physician supervision required to bill for physician supervision and attendance, CPT 99183, in a hospital partnered wound clinic? I read that hyperbaric oxygen therapy is considered an 'incident to' service. In an office setting that would require the physician to be in the office suite not just on the hospital campus, would this type of setting require the same or only that he be present on the hospital campus?  
Answer# 68   We have a gentleman who has a wound in a graft site. He was grafted 15 years ago, and the patient states that the graft never did heal completely. He also continues to get new openings. Because of the length of time since his graft can we consider this a failed graft? We also have a burn patient who was grafted in 1986. She is having similar problems. Can these cases be considered for HBO?
